1. Last name, first name, patronymic (if available), date of birth of the debtor – individual, or the name, identification code of the legal entity in the Unified State Register of Legal Entities, Individual Entrepreneurs, and Public Formations of the debtor – legal entity;
2. The name of the authority or the last name, first name, patronymic, and position of the official who issued the enforcement document;
3. The name of the state enforcement service or the last name, first name, patronymic of the private executor, contact number, and email address of the executor;
4. The number of the enforcement proceedings;
5. The category of enforcement (alimony, fines, etc.).

You can check for your name in the debtor register completely free of charge online. To do this, you need to:
Step 1. Go to the website of the Ministry of Justice of Ukraine.
Step 2. Enter your details – first name, last name, patronymic (if available), tax identification number, and date of birth.
Step 3. Get the result – the system will show whether you are on the debtor list and for which enforcement proceedings.
